Parent Survey 18-19

Dear Parent,

We need your input! Our school district and the Kentucky Department of Education (KDE) want to hear from parents regarding how well your child’s school is doing to involve you in the special education services provided to your child.

We would like you to complete a brief, online survey to share your experience with your child’s special education services during the current school year (2018-19). Please go to www.kypso.org, click “Click here to access the 2019 Parent Survey,” and complete the eleven-question survey telling us about your current experiences.

If you would like to visit the school to complete the survey, you are more than welcome to use one of our computers. Unfortunately, this survey is only available electronically as we are unable to process paper copies.

Only one parent per student should complete this survey. However, if you have more than one child with an IEP, please complete a survey for each of your children.

All responses are anonymous and cannot be traced back to you or your child. Results will be analyzed by the Human Development Institute (HDI) at the University of Kentucky (UK) and used for a report to the U.S. Department of Education.  We will also be reviewing the final report to see if we can do more to involve parents in their child’s special education program.

The deadline for completing this very important survey is May 31, 2019.

Estimado Padre de Familia,

¡ Necesitamos su opinión! Su distrito escolar y el Kentucky Department of Education (KED) (Departamento de Educación de Kentucky) desean saber lo que los padres tienen que decir, acerca de qué tan bien la escuela de su hijo o hija está facilitando el que usted colabore en los servicios de educación especial proporcionados a sus hijos.

Deseamos que complete una breve encuesta en línea, para que comparta su experiencia con los servicios de educación especial de su hijo o hija, durante el presente año escolar (2018-2019).

Por favor diríjase a www.kypso.org, pulse “Pulse Aquí para Accesar la Encuesta para Padres de Familia del Año 2019” y complete la encuesta de 11 preguntas para compartir con nosotros sus experiencias actuales.

Si usted desea visitar la escuela para completar la encuesta, le invitamos a utilizar una de nuestras computadoras. Desafortunadamente, esta encuesta solamente está disponible de manera electrónica, ya que no podemos procesar copias en papel impreso.

Un solo padre debe completar la encuesta de cada estudiante. Sin embargo, si tiene más de un hijo o hija con un Individualized Education Program (IEP) (Programa de Educación Individualizado), favor de completar un cuestionario por cada uno de sus hijos. Todas las respuestas son anónimas y no pueden identificarle a usted, o a su hijo o hija.

EDUCATION OF STUDENTS WITH DISABILITIES

Each school in Bourbon County School System has programs which help to remediate learning problems. The Bourbon County Board of Education is committed to the principle of an appropriate education for all children regardless of their abilities, and that all children are entitled to learn. Before a pupil is placed in a special program written consent of the parents is required.

 

CHILD FIND

Bourbon County’s exceptional education program ensures the provision of appropriate educational and related services in the least restrictive environment for each student identified as have a disability – as defined by The Individuals with Disabilities Education Act (IDEA). “Special” programs and/or services are available for those students who have been identified as having one or more of the following disabling conditions: Autism, Deaf-Blindness, Developmental Delay, Emotional/Behavioral Disability, Functional Mental Disability, Hearing Impairment, Mild Mental Disability, Multiple Disability, Orthopedic Impairment, Other  Health Impairment, Specific Learning Disability, Speech Or Language Impairment, Traumatic Brain Injury, and Visual Impairment.      

          

Any person in Bourbon County who has knowledge of any child with a disability, or who is suspected of having a disability and is not currently receiving special services is encouraged to call the Bourbon County Director of Special Education at (859) 987-2180. Children between the ages of 3 to 21 years qualify for special services. Federal Child Find Laws mandate the Bourbon County Public School System to actively find children with, or suspected of having, a disability.
